We have about 200 members and each of them have different numbers of badges earned. And here is a screenshot of one of the members that has earned some of the badges – http://d.pr/i/1haP7/4RJ4vS1d

Any suggestions on what we could try to get the Reports Add-on working?

Yes, I believe we have a few hundred badges that have already been earned. I’m not sure if it’s just not capturing the approvals? I’m not seeing any data in the logs either.
